Case Manager - Primary Care

What is the Role of the Outreach Case Manager?

The role of the Community Outreach Case Manager is to bring the quality healthcare services of the clinic to underserved populations by building bridges between the patient and the provider.

 

Requirements:

  • BA/BS in psychology, social work or related clinical field
  • 1 year experience with target population.

 

Essential Functions:

  • Demonstrate the ability to build trust relationships with members of the community from all demographic groups that the clinic interacts with.
  • Demonstrate the ability to plan, lead and follow up on educational events.
  • Provide patients with the support that they need to successfully navigate area social services in order to fulfill their needs.
  • Assist members of underserved populations to enroll and maintain participation in health and social service safety net programs.
  • Advocate on behalf of underserved populations in an effective manner.
  • Build strong partnerships with and among health and social service organizations frequently accessed by underserved populations

 

Job Type: Full-time, Non-Exempt

Location: Hammond, IN

Salary: $17.26 - $22.43/hour
